I'm assuming you have the printed copy of this pastpaper.
First measure the length between the object and the lens

Write down the ans in 4 (a)

Then Measure the Diameter ^ and the maximum thickness ^ of the lens

Calculate average by calculating around 2-3 values of the diameter and divide by the number of values you have taken.
Write down thickness of lens.

Make a diagram like this ^
Place the lens between the two pieces of wood and take measurement. First take measure of the wood - Then measurement of the whole wood + wood + lens then deduct the calculation of the wood + wood. You'll get the answer for the lens.
Use the formula and the given values to get the answer for the (IV)

See the answer you get and try to answer this yourself, let me know if you can't (part V)
